Instruction on filling the form

(a) Download the relevant form (Application form and Reference)
(b) Type in your details in UPPER CASE letters using font size 10
(c) Print out the form as filled by you
(d) Sign the document
(e) For students filling the application form, attach your passport photograph, where applicable. This step does not apply to the Referees
(f) Scan the form(s) as a PDF file and email along with supporting documents to wascal_cchh@futminna.edu.ng and capacitybp@wascal.org
